Completar base de datos con formulario.

Hola a todos, les explico mi situación:

1.- En el archivo Adjunto se presiona el botón "FActuración" de la Hoja "CUADRO", una vez presionado se ejecuta un formulario que contiene los siguientes datos.

REPORTE DE QX______________

REMISIÓN_______________

FACTURA_______________

2.- Se requiere que al colocar un valor correspondiente a la columna "N" (REPORTE DE QUIRÓFANO) me habilite los textbox "Remisión" Y "Factura" para capturar los datos correspondientes a ese reporte.

3.- Estos datos deberan guardarse en las columnas "O" y "P" de cada reporte requerido.

Espero haberme explicado correctamente y agradezco su apoyo como siempre.
